Telehealth Services: Telehealth services are a form of healthcare provided via electronic communication and information technologies. This includes audio and video communication and may include the electronic exchange of medical information.

Benefits and Limitations: Telehealth services offer benefits such as improved access to care and convenience but also have limitations such as the inability to conduct a physical examination or provide emergency services.

Privacy and Security: We take your privacy and security seriously and use appropriate technical and administrative safeguards to protect your information. However, please remember that electronic communications may be subject to interception or loss and cannot be guaranteed secure.

Diagnosis and Treatment: Telehealth services provide access to healthcare providers for diagnosis, treatment, and prescription of medications when appropriate. Your healthcare provider may determine that an in-person examination or referral to a specialist is necessary.

Risks and Alternatives: Telehealth services have potential risks, including but not limited to technical difficulties, inadequate evaluation due to the lack of a physical examination, and delays in treatment. Alternative forms of healthcare delivery are available, such as in-person visits or referrals to specialists.

Records: Your telehealth consultation will be documented in your medical history and subject to the same confidentiality laws and regulations as in-person visits.

At The Medicology, Dr. Hector Varas and his medical staff understand that your health information is very personal. We are mandated by the Health Insurance Portability and Accountability Act (HIPAA) to protect your health information. Therefore, we create a record of the care and services you receive from us, which helps provide you with quality care and to comply with specific legal requirements. This Notice applies to all forms of your care generated by us and informs you about how we may use and disclose information about you. We also want to let you know that we also describe your rights to the health information we keep about you and our obligations regarding using and disclosing your health information.

Your Medical Records: The original copy of your electronic medical record is the property of Medicology and Dr. Hector Varas. You may request a copy of your documents to be transferred by completing a medical records release form. We would like 14 business days from your request to prepare and send your records unless the records are urgent or for life-threatening health issues.

Changes to this Notice: We reserve the right to change this Notice. We will post a copy of the current notice in our facility with the current effective date.

Complaints: If you believe your privacy rights have been violated, you may file a complaint with us. So that you know, all complaints must be in writing. Permission to Share your Health Information: We must follow specific federal guidelines and laws regarding the confidentiality of your personal health information. One prevents us from discussing anything in your medical file with anyone other than yourself or other medical personnel involved in your care. So, if you would like us to discuss lab results or additional personal information with your significant other, family members, or any other individuals, please let us know their names at your appointment.

I understand and agree that all services rendered are charged directly to me and that I am responsible for payment. In case of non-payment, I also agree to bear the cost of collection, court costs, and reasonable legal fees, should this be required. I understand that I am not authorized to initiate a chargeback or dispute for any transaction that has been processed by Medicology LLC unless there is a legitimate reason for such a claim. I also understand that if I have agreed to recurring payments, I am responsible for ensuring that sufficient funds are available in my account at each payment. I want to let you know that failure to do so may result in additional fees, collection attempts, and suspension or termination of the service. I can cancel my subscription anytime but I must give a 15-day cancellation notice. If you have a prepaid subscription, we will not issue any refunds for the unused portion of the subscription. However, you will continue to have access to the subscription until the end of the prepaid period. Finally, I understand that Medicology has the right to take any necessary action to recover any outstanding fees or charges owed, including but not limited to the use of collection agencies and legal action.
